compresses for 3+ years now and it's well worth it. (Though now doing once daily; after you get things under control, you can usually go to one a day.

Sounds like eczema, it can crop up as just isolated patches. Try an OTC corticosteroid cream ++
or if that doesn't help then ask your pharmacist or doctor for recs. Argan oil is very soothing and healing for my skin, but YMMV I've had eczema
